The Physical Therapist is a member of the home care team who treats patients in their own homes, based on the Medicare part b outpatient model as prescribed by their doctor.

  1. Licensed Physical Therapist in the state.
  2. Can commit to a minimum of 5 cases (10 hours a week).
  3. Complies with accepted professional standards and practice.
  4. Possesses and maintains current Liability Insurance.

  3. Can commit to a minimum of 5 cases (10 hours a week).
